USA Hockey Consent to Treat Form
 
LBYH is requesting all players fill out a Consent to Treat form.  This allows LBYH to seek treatment for a player injured on the ice or otherwise at the rink.  While submitting this form it is not mandatory to play in the program, it is highly recommended, as most parents do not attend every game and practice.  
 
Team Managers should distribute this two-page form to each family and collect the completed forms.  In order for coaches to have access to this form at each game and practice, we ask that Managers create two sets and place them each in a folder and give one to the head coach and one to an assistant coach.  Then submit the ‘master’ forms to Maura Fiske ( ) for the season.   Please make your parents aware that at the end of the season all masters and copies will be destroyed.
 
Note:  Along with the Consent to Treat form for each player, every team should also have the current official, stamped USA Hockey Roster and emergency contact Information for every player in the folder for all games and practices.


MA Hockey Player Patch Form

A player is eligible for a Player Patch if he/she contributes 3 Goals or 3 Assists, or the Goalie allows no Goals to be scored in one game.  Please fill out the MA Hockey Award form, attach a copy of the game sheet and mail it to the contact listed on the form.  


MA Hockey Application for Travel Permit

Travel permits must be submitted when your team will be leaving Massachusetts for an overnight tournament, except for any Valley League games.  Once the form has been completed forward it to the LBYH Registrar.  He/she will submit it to the MA Hockey Registrar together with a copy of your team’s Stamped Roster.  It is a good idea to allow at least a couple weeks for the District Registrar to process the request.
 
Note: There's no fee for a travel permit. However, approval of a travel permit application is dependent on the district having signed rosters for that team. So, if there are any teams doing an out of state tournament early in the season then they need to do their rosters before then.


MA Hockey District Playdown Application Form
 
This form is for the Mass Hockey State Tournament.  Be sure your coach has filled out the form prior to State Playdowns and has forwarded it to the LBYH Registrar.  Be sure to have a copy of the USA Hockey Stamped Roster and the Playdown Form.
